
• Make an Appointment
To arrange for your personal evaluation contact my office at 216-778-2262. To obtain more information about scheduling or procedures, I encourage you to complete the Patient Contact Form. You will receive a response from my office by e-mail or phone at your request.
• About Your First Visit
I request that you print, complete and bring the Patient Information Form to your visit. If you are a weight loss patient, I also ask that you print, complete and bring the Weight Loss Patient Form to your visit. After obtaining your medical history and reviewing your concerns, the appropriate physical examination will be performed. If medical photography (see below) is necessary, the photos will be taken just after your exam. You will then be allowed to dress, and we will spend the rest of your visit discussing options for you.
• Consultation Fee
The consultation fee for a new patient visit is $65 if payment is made at the time of scheduling your appointment. The fee is $90 if paid at the time of the visit. This consultation fee covers your initial evaluation (and a second office if necessary) and will be applied to the cost of surgical procedure scheduled within 6 months of the initial visit.
• Cancellations
Please notify my office 24 hours prior to your appointment if you need to cancel. If you are more than 20 minutes late for your appointment you may be asked to reschedule to another date.
• Photography
Clinical photographs are routinely obtained to document your pre-operative appearance and assist in surgical planning. Photographs may need to be sent to your insurance company (if applicable) in support of a request for authorization for a covered procedure under your policy. You will be asked to sign a Photography Release Form at your first visit. These photographs will be used for documentation and surgical planning only. Your images will not be used for publication without your express written consent.
• Insurance
If you are seeing me for an insurance covered procedure or a combined insurance covered and cosmetic procedure, please bring your insurance card to your visit. For most insurance covered procedures, we are required to provide clinical information to your insurance carrier and request authorization- a process known a pre-determination. Depending on the individual company this may take 1 to 8 weeks to complete. For more information see Combined Surgical Procedures on the Other Information page
